Revita Dentistry Accepts the Canadian Dental CarePlan
Looking for a CDCP dentist in Brampton? Revita Dentistry welcomes patients covered by the Canadian Dental Care Plan (CDCP).
We can help you understand your available coverage, explain your co-payment and potential out-of-pocket costs before treatment, and submit eligible CDCP claims directly to Sun Life. Our team is here to make your dental visit straightforward, comfortable, and well explained.
What Is the Canadian Dental Care Plan?
The Canadian Dental Care Plan is a Government of Canada program that helps make oral health care more affordable for eligible Canadian residents who do not have access to private dental insurance or coverage.
The CDCP does not automatically cover every cost or every treatment. It pays for eligible services at established CDCP fees, and some patients may have a co-payment or additional costs.

Who May Be Eligible for CDCP?
You may qualify for the CDCP if you meet all of the following conditions:
- You do not have access to private dental insurance or coverage—including a health spending account that covers dental costs—through your work, pension, a family member’s plan, a professional or student organization, or privately purchased coverage.
- You and your spouse or common-law partner, if applicable, have filed your Canadian tax returns for the previous year.
- Your adjusted family net income is below $90,000.
- You are a Canadian resident for tax purposes.
If you receive dental benefits through an eligible government social program, you may still qualify. Coverage may be coordinated between programs.

CDCP Coverage and Co-Payments
Your level of coverage is based on your adjusted family net income.
| Adjusted family net income | CDCP pays | You pay |
|---|---|---|
| Less than $70,000 | 100% of eligible services at CDCP established fees | 0% co-payment, plus any additional charges if applicable |
| $70,000 to $79,999 | 60% of eligible services at CDCP established fees | 40% co-payment, plus any additional charges if applicable |
| $80,000 to $89,999 | 40% of eligible services at CDCP established fees | 60% co-payment, plus any additional charges if applicable |
Important: You May Still Have a Balance
CDCP fees may differ from the fees charged by a dental office. Even if your CDCP co-payment is 0%, you may have an out-of-pocket cost if:
- The treatment cost is higher than the CDCP-established fee.
- Your treatment has a required co-payment.
- The service is not covered by the CDCP.
- A recommended service requires preauthorization and is not approved.
Before treatment begins, our team will explain your expected costs and answer your questions.

What Dental Services May Be Covered?
The CDCP may help cover a broad range of eligible oral health care services when recommended by an oral health provider. Coverage, frequency limits, and preauthorization requirements vary by service and individual circumstances.
Eligible services may include:
- Dental exams and X-rays
- Cleanings, fluoride treatments, and sealants
- Fillings and other basic restorative care
- Root canal treatment and other endodontic services
- Gum disease treatment, including certain periodontal services
- Complete or partial removable dentures and denture repairs
- Tooth and root extractions, as well as certain oral surgery services
- Nitrous oxide, oral sedation, and other eligible sedation services
Some services require preauthorization before treatment. Certain services may also require preauthorization when they exceed CDCP frequency limits.
Orthodontic services are not currently available under the CDCP.
Do I Need Preauthorization?
Some treatments require CDCP preauthorization before they can be confirmed for coverage. This means the treatment must be reviewed against the plan’s clinical criteria before it is provided.
If preauthorization is needed, we will discuss it with you and explain the next steps. Approval is not guaranteed, and you can decide whether to proceed with treatment if it is not covered.

What to Bring to Your Appointment
When booking your first CDCP appointment, please have:
- Your CDCP Member ID
- Your benefit coverage start date
- Your co-payment level
- Government-issued photo identification
You do not need to wait for your physical Sun Life member card to receive care. However, your CDCP coverage must be active, and appointments should be booked on or after your coverage start date.
How to Apply for CDCP
You can apply for the Canadian Dental Care Plan through the Government of Canada.
You may also be able to apply by phone or at a Service Canada Centre. For application support and the latest eligibility information, visit Canada.ca/dental.

What Happens After You Apply?
If your application is approved, you will receive confirmation of your CDCP enrolment. This information includes your Member ID, coverage start date, and co-payment level.
Sun Life will also send a welcome package with your member card and plan information. Before your appointment, contact our office so we can help verify your coverage and discuss any expected patient portion.
CDCP members must renew their coverage each year to confirm they remain eligible and avoid a gap in coverage.
